Remove annotations strips every annotation from every page of a PDF in a single pass: sticky notes and their pop-ups, highlights, underlines and strike-throughs, freehand ink, shapes, stamps, free text boxes, links and the widgets of form fields. What is printed on the page, text, images and vector graphics, is left exactly as it was. The document is processed in your browser and never uploaded.

The tool deletes the whole annotation list of each page and the document's form definition, then saves the file. Yes. Form fields are annotations, so they are removed along with anything typed into them. If you need the answers to remain visible, flatten the form with Flatten PDF instead of removing the annotations.
The signature widget is removed and the file is rewritten, so the signature is lost. Keep the signed original and clean a copy if you need both.
